Our certified hospice aides assist patients with personal care needs while maintaining dignity and comfort. Care is RN guided and care planned around the individuals specific needs.

• Dressing assistance
• Skin care
• Bed changes
• Hygiene support
• Safety monitoring

These visits provide both physical assistance and meaningful companionship.

Serious illness affects the entire family, not just the patient.

Our licensed social workers help families navigate the emotional and practical challenges that come with serious illness by assisting with:

• Emotional counseling
• Family communication
• Advance care planning
• Caregiver support
• Community resources

Our goal is to help families feel supported, informed, and never alone.

For many individuals and families, spiritual care becomes especially meaningful during serious illness.

Our chaplains provide compassionate support regardless of religious background or beliefs. 

Spiritual services may include:

• Prayer and spiritual counseling
• Meaning-centered conversations
• Support for grief and existential concerns
• Connection with faith communities

All spiritual services are completely optional and guided by patient preference.

Hospice care includes the medications, equipment, and supplies necessary to manage the patient’s hospice diagnosis.

This may include:

• Pain and symptom control medications
• Hospital beds
• Oxygen equipment
• Wheelchairs and walkers
• Wound care supplies
• Incontinence supplies

Our team coordinates delivery and setup so families can focus on spending time together.

Community First Hospice Care provides 24-hour access to a nurse; thus, ensuring families always have someone to call when questions or concerns arise.

Whether it is the middle of the night or a weekend, our team is ready to help.

Hospice care is delivered through a team of dedicated professionals who work together to meet the physical, emotional, and spiritual needs of each patient.

We offer grief and bereavement support for up to 13 months following a patient’s death.
