At Devoted Care Hospice, we understand the complexity and urgency that often surrounds hospice referrals. Our support begins the moment we receive a patient’s name. From that point forward, our team works to simplify the process; coordinating communication, guiding families, and helping navigate each step of hospice eligibility and admission. Our goal is to make referrals seamless for healthcare professionals while ensuring patients and families receive timely, compassionate support. 

Here are some ways we go about easing the process: 

Clear and honest explanation of the hospice benefit and what the individual is entitled to through their insurance benefit. Allow us to educate on hospice services to aide in making informed decisions around care. You don't have to elect hospice to learn about the benefit. 

Whether a patient is considering a return home or seeking placement in an assisted living facility, personal care home, or other facility-based care site, we're here to guide and support that decision with a diverse book of referral options and experience in what such transitions look like for the patient and family. 

Access to and delivery of durable medical equipment, such as hospital beds, oxygen, wheelchair, and other supplies, within 2-4 hours of request. 

Individualized and advance care planning, because each patient and their needs are unique. Our care planning incorporates the patient, family, their history, and the referring providers insight to ensure a thoughtful, meaningful, and productive plan is in place. 

We offer volunteer opportunities for individuals who want to make a difference in the lives of others. Our volunteers provide companionship, emotional support, and assistance with daily living activities.
